Indications
- Congestive heart failure
- Hepatic cirrhosis with ascites and edema

Description
Long-acting aldosterone antagonist. Specific pharmacologic antagonist of aldosterone. Causes increased excretion of sodium and water, while retaining potassium and magnesium.
Pharmacology
Aldosterone antagonist, acts as a diuretic and antihypertensive drug. Binds to mineralocorticoid receptor, blocking actions of aldosterone on gene expression.
Dosage & Administration
- Initial daily dosage of 100 mg for edema in adults
- Doses of 100 to 400 mg for primary hyperaldosteronism
- Initial daily dosage of 50 to 100 mg for essential hypertension
- Dosage ranging from 25 mg to 100 mg for hypokalemia
Interaction
- Concomitant administration of ACE inhibitors with potassium-sparing diuretics has been associated with severe hyperkalemia
- Potentiation of orthostatic hypotension may occur with alcohol, barbiturates, or narcotics
- Intensified electrolyte depletion may occur with corticosteroids, ACTH
- Lithium generally should not be given with diuretics
- Verospiron has been shown to increase the half-life of digoxin
Contraindications
Contraindicated in patients with acute renal insufficiency, significant impairment of renal function, anuria, hyperkalemia, or sensitivity to Spironolactone.
Side Effects
- Gynaecomastia
- GI symptoms including cramping and diarrhea
- Drowsiness
- Lethargy
- Headache
- Urticaria
- Mental confusion
- Impotence
- Irregular menses or amenorrhea
- Post-menopausal bleeding
Pregnancy & Lactation
- Spironolactone should not be used during pregnancy
- Canrenone, an active metabolite of Spironolactone, appears in breast milk
Precautions & Warnings
All patients receiving diuretic therapy should be observed for evidence of fluid or electrolyte imbalance. Hyperkalemia may occur in patients with impaired renal function or excessive potassium intake.
Overdose Effects
Symptoms include drowsiness, mental confusion, dizziness, diarrhea, and vomiting. Patients should induce vomiting or evacuate the stomach by lavage during Verospiron overdose.
