Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
- For adults, elderly, and children over 1 year

- 0.05% w/w concentration of Clobetasol Propionate BP
Pharmacology
- Very potent topical corticosteroid
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive properties
- Inhibition of late phase allergic reactions
- Reduction in mast cell density
- Inhibition of eosinophil activation
- Reduction in cytokine production
- Inhibition of arachidonic acid metabolism
Dosage
- Application of thin layer twice daily for adults, elderly, and children over 1 year
- Potential for enhanced effect by occluding treatment area with polythene film
- Super-high potency topical corticosteroid
- Limited treatment duration to 2 consecutive weeks
- Maximum weekly dose of 50 gm/week
- Use limited to 5 days in children
- Apply required quantity of spray once or twice daily to affected areas of the scalp
- Limit scalp treatment to 4 consecutive weeks
Administration
- Cutaneous route of administration
- Creams for moist or weeping surfaces
- Ointments for dry, lichenified, or scaly lesions
Interaction
- Inhibition of corticosteroid metabolism by co-administered drugs such as ritonavir and itraconazole
- Resulting in increased systemic exposure
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for primarily infected skin lesions caused by fungi or bacteria
- Not recommended for dermatoses in children under one year of age
Side Effects
- Systemic absorption resulting in hypercorticism features
- Local atrophic changes with prolonged and intensive treatment
- Rare instances of provoking the pustular form of psoriasis
- Reports of pigmentation changes and hypertrichosis
- Well-tolerated with possibility of hypersensitivity
Pregnancy & Lactation
- Limited data from use in pregnant women
- Potential for foetal development abnormalities
- Consideration of risks versus benefits during pregnancy and lactation
Precautions & Warnings
- Avoid long-term continuous therapy, especially in infants and children
- Risk of atrophic changes on the face after prolonged treatment
- Avoid application to the eye lids to prevent glaucoma or cataract
- Hazardous effects in psoriasis, careful patient supervision needed
- Use anti-microbial therapy for infected inflammatory lesions
- Cleansing required before applying fresh dressing
Use in Special Populations
- Avoid long-term continuous therapy in infants and children under 12 years
Overdose Effects
- Unlikely acute overdosage
- Features of hypercortisolism in cases of chronic over-dosage
