Indications
- Edema associated with congestive heart failure
- Edema due to various forms of renal dysfunction
- Hypertension
Pharmacology
- Promotes water loss from the body
- Inhibits Na+/Cl- reabsorption from the kidneys
- Causes loss of potassium and increase in serum uric acid
- Prevents hypertension-related morbidity and mortality
Dosage & Administration
- Initial dose for edema: 25 to 50 mg daily
- Maximum daily dose for edema: 100 mg
- Initial dose for hypertension: 25 mg daily
- Maximum daily dose for hypertension: 50 mg
Interaction
- Potentiation of orthostatic hypotension with alcohol, barbiturates, or narcotics
- Impaired absorption with cholestyramine and colestipol resin
- Interference with tests for parathyroid function
Contraindications
- Anuria
- Hypersensitivity to Hydrochlorothiazide
- Severe renal or hepatic failure
- Addison’s disease
- Concurrent lithium therapy
Side Effects

- Leucopenia
- Yellow vision
- Renal dysfunction
- Rash
- Hyperglycaemia
Pregnancy & Lactation
- Crosses placental barrier
- Appears in breast milk
- Not recommended in otherwise healthy pregnant women
- Patient should stop breast-feeding if essential
Precautions & Warnings
- Monitoring for signs of fluid and electrolyte imbalance
- Warning signs of imbalance include dryness of mouth, muscle cramps, and hypotension
- Oliguria may occur when creatinine clearance falls below 30ml/min
Overdose Effects
- Electrolyte depletion
- Dehydration
- Accentuation of cardiac arrhythmias with digitalis administration
