Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Stimulates release of aldosterone
- Selective AT 1 receptor blocker
- Does not inhibit ACE (kininase II)
- Inhibits negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- Dose may be increased to 40 mg if further reduction in blood pressure required
- No initial dosage adjustment for elderly patients or patients with moderate to marked renal impairment
- May be administered with or without food
Interaction
- No significant drug interactions reported in studies
- Not metabolized by cytochrome P450 system
- Caution advised with co-administration of NSAIDs and diuretics
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
Side Effects
- Well tolerated with similar incidence of adverse reactions to placebo
- Adverse reactions not dose-related
- Common adverse reactions include dizziness, back pain, diarrhea, headache, hyperglycemia, rhinitis
Pregnancy & Lactation
- Discontinue use when pregnancy detected
- Avoid use in the second and third trimesters
- Potential excretion in human milk

- Changes in renal function may be anticipated
- Treatment associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals

- Most likely effects include hypotension and tachycardia
