Indications

  • Prophylaxis and/or treatment of thromboembolic complications associated with atrial fibrillation and/or cardiac valve replacement
  • Indicated to reduce the risk of death, recurrent myocardial infarction, and thromboembolic events
  • Prophylaxis and treatment of venous thrombosis and pulmonary embolism
  • Transient ischaemic attacks

Description

  • Act as an anticoagulant
  • Inhibits the synthesis of vitamin K dependent clotting factors
  • Affects clotting factors II, VII, IX and X

Pharmacology

  • Acts as a vitamin K antagonist
  • Inhibits the production of vitamin K by vitamin K epoxide reductase
  • Affects coagulation factors VII, IX, X, and thrombin

Dosage & Administration

  • Usual adult induction dose of 10 mg daily for 2 days
  • Subsequent maintenance dose depends on prothrombin time (INR)
  • Adult maintenance dose usually between 3 to 9 mg taken daily
  • Initial dose for children usually 0.1 mg.kg-1.d-1

Interaction

  • Has a greater potential for clinically significant drug interactions
  • Warn patients about potential hazards of drug interactions

Contraindications

  • Cannot be administered in actual or potential hemorrhagic conditions
  • Cannot be administered to patients with uncontrolled hypertension
  • Cannot be administered in cases of severe hepatic or renal disease
  • Cannot be administered during pregnancy
  • Cannot be administered to patients with known hypersensitivity to warfarin
  • Cannot be administered in cases of bacterial endocarditis

Side Effects

  • Principal adverse effect is hemorrhage
  • Other adverse reactions include nausea, vomiting, diarrhea, rash, and alopecia

Pregnancy & Lactation

  • Contraindicated in the first trimester of pregnancy due to risk of teratogenicity
  • Should not be used in women who are or may become pregnant
  • Appears in the milk of nursing mothers in an inactive form

Precautions & Warnings

  • Periodic determination of prothrombin time (PT)/international normalized ratio (INR) is essential
  • Response to anticoagulants may be influenced by travel, changes in diet, and medication
  • Monitor patient's response with additional PT/INR determination after discharge from the hospital
  • Several factors may exaggerate the effects of warfarin and necessitate a reduction in dosage
  • Factors may call for an increase in maintenance dosage including weight gain, diarrhea and vomiting
  • Reversal of warfarin anticoagulation by vitamin K takes several days

Overdose Effects

  • Excessive depression of coagulation activity can be corrected by temporary withdrawal of warfarin
