Pharmacology

• Proton pump inhibitor
• Suppresses gastric acid secretion
• Specific inhibition of the H+/K+-ATPase

Absorption

• Enteric-coated pellet formulation
• Peak plasma levels occur at approximately 1.5 hours

Distribution

• 97% bound to plasma proteins
• Constant plasma protein binding over a certain concentration range
• Apparent volume of distribution at steady state in healthy volunteers is approximately 16 L

Metabolism

• Extensive metabolism in the liver by the cytochrome P450 (CYP) enzyme system
• Major part of metabolism dependent on the CYP2C19 isoenzyme

Excretion

• Plasma elimination half-life of approximately 1–1.5 hours
• Less than 1% of parent drug excreted in the urine
• Approximately 80% of an oral dose excreted as inactive metabolites in the urine

CombinationTherapy

• Triple therapy to reduce the risk of duodenal ulcer recurrence

Administration

• Tablet or capsule should be swallowed whole and taken one hour before a meal
• For oral suspension, the whole contents of the packet should be taken into a small glass containing water

Interactions

• Extensively metabolized in the liver by CYP2C19 and CYP3A4
• Not likely to inhibit certain CYP enzymes
• Potential interference with CYP2C19
• Potential interference with the absorption of certain drugs where gastric pH is important for bioavailability

• Known hypersensitivity to any of the formulation

PregnancyAndLactation

• No adequate and well-controlled studies in pregnant women
• Animal studies have revealed no teratogenic effects

• Symptomatic response to therapy does not preclude the presence of gastric malignancy
• Tablet or capsule should be taken at least one hour before meals

• Safety and effectiveness in paediatric patients have not been established
• No overall differences in safety and efficacy observed between the elderly and younger individuals

OverdoseEffects

• Signs of acute toxicity include reduced motor activity, changes in respiratory frequency, tremor, ataxia, and intermittent clonic convulsions
