Description
- A combination of bisoprolol and amlodipine indicated for the treatment of hypertension
- Consists of a dihydropyridine calcium antagonist and a beta1-selective adrenoceptor blocking agent
Indications
- Treatment of hypertension as substitution therapy
- Adequate control of blood pressure with individual products given concurrently
Pharmacology
- Inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle
- Reduces peripheral vascular resistance and blood pressure
- Beta1-selective adrenoceptor blocking agent with low affinity to beta2 receptors
Dosage & Administration
- One tablet once daily for patients with adequately controlled blood pressure
Interactions
- Not recommended with calcium antagonists of verapamil and diltiazem type
- Combinations to be used with caution with strong or moderate CYP3A4 inhibitors, simvastatin, tacrolimus, etc.
- Combinations to be considered with mefloquine, rifampicin, ergotamine derivatives, MAO inhibitors
Contraindications
- Acute heart failure or during episodes of heart failure decompensation
- Obstruction of the outflow tract of the left ventricle
- Sinoatrial block, symptomatic bradycardia, severe bronchial asthma
- Untreated phaeochromocytoma, hypersensitivity to bisoprolol, amlodipine, or any excipients
Side Effects
- Common: Dizziness, headache, edema, fatigue
- Uncommon: Insomnia, mood changes, visual disturbances, dyspnea, myalgia
- Rare: Allergic reactions, nightmares, confusion, increased triglycerides
Pregnancy & Lactation
- Not recommended for use
Precautions & Warnings
- Caution in patients with heart failure, impaired hepatic function, or obstructive airways diseases
- Considerations for patients with diabetes mellitus, first degree AV block, Prinzmetal’s angina, psoriasis, thyrotoxicosis
- Advice for patients undergoing general anesthesia
- Gradual withdrawal before surgery
Use in Special Populations
- Caution for geriatric use, no established safety and efficacy for pediatric use
- Special caution in hepatic or severe hepatic impairment, renal impairment, or undergoing dialysis
Overdose Effects
- Signs of beta-blocker overdose: bradycardia, hypotension, bronchospasm, hypoglycemia
- Signs of amlodipine overdose: excessive peripheral vasodilation, reflex tachycardia, systemic hypotension
- Discontinuation of treatment and supportive, symptomatic treatment recommended
