Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain

Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibition of reuptake of neurotransmitters- serotonin and noradrenaline
- Modification of pain impulse transmission
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Sustained release capsule or tablet dosage every 12 hours
- Injection dosage of 50-100 mg every 4 to 6 hours
- Suppository dosage of 100 mg Tramadol Hydrochloride 6 hourly
- Maximum total daily dosage by mouth should not exceed 400 mg
Interaction
- Monoamine oxidase (MAO) inhibitors interaction
- Concomitant administration of carbamazepine causing increased Dolonil metabolism
Contraindications
- Hypersensitivity to Tramadol
- Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids, or psychotropic drugs

Pregnancy & Lactation
- Safe use in pregnancy not established
- Tramadol crossing the placenta
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies risk to the fetus
- Tramadol and its metabolites detected in breast milk
Precautions & Warnings
- Respiratory depression risk
- Not recommended for patients dependent on opioids
- Cautious use with concomitant CNS depressants
- Cautious use in patients taking MAO inhibitors
- Cautious use in patients with increased intracranial pressure or head injury
- Cautious use in patients with acute abdominal conditions
Use in Special Populations
- Dose of 1-2 mg/kg body weight in children from the age of 1 year
- Suppository not to be administered in children and adolescents below the age of 14 years
- Safety and efficacy of Dolonil 100 mg SR Capsules not established in children
