Indications
- Treatment of moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits the reuptake of neurotransmitters
- Modifies transmission of pain impulses
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses 50 to 100 mg every four to six hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Total daily dosage by mouth should not exceed 400 mg
- Sustained Release Capsule or Tablet dosage every 12 hours
- Injection dosage of 50-100 mg every 4 to 6 hours
- Suppository dosage of 100 mg Tramadol Hydrochloride 6 hourly
- Higher daily doses for cancer pain and severe pain after operations
Interaction
- No concern about most drugs interacting with Dolonil SR
- Monoamine oxidase (MAO) inhibitors not recommended for combination with Dolonil SR
- Concomitant administration of carbamazepine causes a significant increase in Dolonil SR metabolism
Contraindications
- Hypersensitivity to the drug
- Acute intoxication with specific substances
Side Effects
- Common side effects: dizziness/vertigo, nausea, constipation, headache, somnolence, vomiting
- Less common side effects: malaise, allergic reaction, weight loss, GI bleeding, hepatitis
- Various other side effects including CNS stimulation, dyspepsia, diarrhea, etc.
Pregnancy & Lactation
- Safe use during pregnancy not established
- Tramadol crosses the placenta
- Not recommended during breastfeeding due to detection in breast milk
Precautions & Warnings
- Cautious administration in patients at risk for respiratory depression
- Not recommended for patients dependent on opioids
- Use with caution and in reduced dosages when administering with CNS depressants
- Use with caution in patients taking MAO inhibitors
- Use with caution in patients with increased intracranial pressure or head injury and patients with acute abdominal conditions
Use in Special Populations
- Can be given in children from the age of 1 year
- Suppository not to be administered in children and adolescents below the age of 14 years
- 100 mg SR Capsules not studied in children
