Indications

  • Edema associated with congestive heart failure
  • Edema associated with hepatic disease
  • Edema associated with renal disease
  • Edema associated with nephrotic syndrome

Pharmacology

  • Loop diuretic
  • Rapid onset of action
  • Short duration of action
  • Equivalent diuretic potency to furosemide
  • Action on ascending limb of the loop of Henle
  • Inhibition of sodium and chloride reabsorption
  • Possible action in the proximal tubule
  • Phosphaturic action
  • No noticeable action on the distal tubule

Dosage & Administration

  • Oral administration
  • Initial dose of 1 mg in the morning
  • Severe cases may require 5 mg daily
  • IV Injection dose of 1-2 mg
  • IV Infusion dose of 2-5 mg over 30-60 minutes
  • IM Injection initial dose of 1 mg
  • Dosage adjustment for elderly patients
  • Paediatric use safety and effectiveness

Interaction

  • Interaction with lithium
  • Interaction with probenecid
  • Interaction with indomethacin
  • Potentiation of antihypertensive drugs' effect
  • Effect on digoxin blood levels
  • No effect on warfarin metabolism

Contraindications

  • Avoid in severe hypokalaemia
  • Avoid in severe hyponatraemia
  • Avoid in anuria
  • Avoid in comatose and precomatose states associated with liver cirrhosis
  • Avoid in renal failure

  • Safety and effectiveness in paediatric patients below the age of 18 not established

  • Reduction of blood volume
