Indications
- Edema associated with congestive heart failure
- Edema associated with hepatic disease
- Edema associated with renal disease
- Edema associated with nephrotic syndrome
Pharmacology
- Loop diuretic
- Rapid onset
- Short duration of action
- Diuretic potency equivalent to furosemide
- Action on the ascending limb of the loop of Henle
- Inhibition of sodium reabsorption
- Inhibition of chloride reabsorption
- Possible additional action in the proximal tubule
- Phosphaturia during diuresis
Dosage & Administration
- Oral dosage: 1mg in the morning, repeated after 6-8 hours if necessary
- IV Injection dosage: 1-2mg, repeated after 20 minutes if necessary
- IV Infusion dosage: 2-5mg over 30-60 minutes
- IM Injection dosage: 1mg initially then adjusted according to response
- Paediatric use: Safety and effectiveness not established
Interactions
- Interaction with lithium
- Interaction with probenecid
- Interaction with indomethacin
- Potentiation of antihypertensive drugs
- No effect on digoxin blood levels
- No effect on warfarin metabolism
Contraindications
- Severe hypokalaemia
- Severe hyponatraemia
- Anuria
- Comatose and precomatose states associated with liver cirrhosis
- Renal failure
Side Effects
- Muscle cramps
- Dizziness
- Hypotension
- Headache
- Nausea
- Other possible effects: ECG changes, musculoskeletal pain, abdominal pain, renal failure, thrombocytopenia
Pregnancy & Lactation
- Pregnancy Category C
- No adequate and well-controlled studies in pregnant women
- Unknown excretion in human milk
Precautions & Warnings
- Periodic measurement of serum potassium
- Potential need for potassium supplements or potassium sparing diuretics
Use in Special Populations
- Safety and effectiveness not established for pediatric patients below 18 years
Overdose Effects
- Acute profound water loss
- Volume and electrolyte depletion
- Dehydration
- Reduction of blood volume
- Circulatory collapse
- Possibility of vascular thrombosis and embolism
- Treatment consists of fluid and electrolyte replacement
