Indications
- Treatment of edema associated with congestive heart failure
- Treatment of edema associated with hepatic and renal disease
- Treatment of edema associated with nephrotic syndrome
Pharmacology
- Rapid onset and short duration of action
- Equivalent diuretic potency to approximately 40 mg furosemide
- Major site of action: ascending limb of the loop of Henle
- Inhibits sodium and chloride reabsorption in the ascending limb
- Possible additional action in the proximal tubule
- No noticeable action on the distal tubule
Dosage & Administration
- Oral dosage: 1 mg in the morning, with possible repeat after 6-8 hours
- IV Injection dosage: 1-2 mg, with possible repeat after 20 minutes
- IV Infusion dosage: 2-5 mg over 30-60 minutes
- IM Injection dosage: 1 mg initially, with adjustment according to response
- Paediatric use: Safety and effectiveness not established below age 18
Interaction
- Contraindicated with lithium and probenecid
- Not recommended with indomethacin
- May potentiate antihypertensive drugs
- No effect on digoxin blood levels
- No effect on warfarin metabolism
Contraindications
- Severe hypokalaemia
- Severe hyponatraemia
- Anuria
- Comatose and precomatose states associated with liver cirrhosis
- Renal failure

- Periodic measurement of serum potassium
- Consideration of potassium supplements or potassium sparing diuretics

- Safety and effectiveness not established in paediatric patients below age 18

- Acute profound water loss
- Volume and electrolyte depletion
- Dehydration
- Reduction of blood volume
- Circulatory collapse
- Vascular thrombosis and embolism
- Treatment: replacement of fluid and electrolyte losses
