Indications

  • Treatment of acute otitis media caused by different microorganisms
  • Treatment of pharyngitis/tonsillitis caused by specific bacteria
  • Treatment of acute maxillary sinusitis caused by certain strains
  • Management of community-acquired pneumonia caused by susceptible bacteria
  • Treatment of acute bacterial exacerbation of chronic bronchitis
  • Management of skin and skin structure infections caused by specific pathogens
  • Management of uncomplicated urinary tract infections
  • Treatment of uncomplicated gonorrhea
  • Management of rectal gonococcal infections in women

Pharmacology

  • Activity against Gram negative and Gram positive bacteria
  • Stability to beta lactamases
  • Prodrug with active metabolite Cefpodoxime
  • Excretion of Cefpodoxime in urine

Dosage & Administration

  • Specific dosages for adults and adolescents for various infections
  • Specific dosages for infants and pediatric patients for certain infections
  • Dosing adjustment for patients with renal dysfunction
  • No dose adjustment needed for patients with liver cirrhosis

Interaction

  • Reduced peak plasma level and absorption with concomitant use of high doses of antacids or H 2 blockers
  • Renal excretion inhibition by probenecid
  • Need for close monitoring of renal function when administered with nephrotoxic drugs

Contraindications

  • Contraindicated in patients with known allergy to cephalosporin antibiotics

Side Effects

  • Occurrence of diarrhea, nausea, skin & vaginal fungal infection, abdominal pain, headache, chest pain, myalgia, dyspepsia, dizziness, vertigo, cough
  • Incidence of fungal skin rash is higher in children than in adults

Pregnancy & Lactation

  • No evidence of teratogenic or embryocidal effects in animal trials
  • Use during pregnancy only if clearly needed
  • Excretion of Cefpodoxime in human milk, requiring a decision on breastfeeding

Precautions & Warnings

  • Dose reduction in patients with reduction in urinary output due to renal insufficiency
  • Caution when administered to patients receiving concurrent treatment with potent diuretics
  • Risk of proliferation of non-susceptible organisms with prolonged use
