Indications
- Treatment of edema associated with congestive heart failure
- Treatment of edema associated with hepatic and renal disease
- Treatment of edema associated with nephrotic syndrome
Pharmacology
- Rapid onset and short duration of action
- Diuretic potency equivalent to approximately 40 mg furosemide for 1 mg Bumetanide
- Major site of action: ascending limb of the loop of Henle
- Inhibits sodium and chloride reabsorption in the ascending limb
- Potential additional action in the proximal tubule
Dosage & Administration
- Oral dosage: 1 mg in the morning, repeat after 6-8 hours if necessary
- Severe cases may require 5 mg daily, increased by 5 mg every 12-24 hours
- Elderly dosage: 500 micrograms daily may be sufficient
- IV Injection dosage: 1-2 mg, repeat after 20 minutes if necessary
- IV Infusion dosage: 2-5 mg over 30-60 minutes
- IM Injection dosage: 1 mg initially, then adjust according to response
- Pediatric use safety and effectiveness not established
Interactions
- Should not be administered concurrently with lithium
- Concurrent therapy with probenecid is not recommended
- Concurrent therapy with indomethacin is not recommended
- May potentiate the effect of various antihypertensive drugs
- No significant effect on digoxin blood levels
- No effect on warfarin metabolism
Contraindications
- Avoid in severe hypokalaemia
- Avoid in severe hyponatraemia
- Avoid in anuria
- Avoid in comatose and precomatose states associated with liver cirrhosis
- Avoid in renal failure

Pregnancy & Lactation
- Pregnancy Category C

- Periodic measurement of serum potassium
- Consider potassium supplements if necessary

- Safety and effectiveness in pediatric patients below the age of 18 not established
