Description
- Metoclopramide is a medication used to treat symptoms such as nausea and vomiting, as well as diabetic gastroparesis. It comes in various forms including nasal spray, tablets, and injections.
Indications
- Relief of symptoms in adults with acute and recurrent diabetic gastroparesis
- Treatment of symptomatic, documented gastroesophageal reflux in adults
- Prevention of nausea and vomiting associated with emetogenic cancer chemotherapy
- Facilitates small bowel intubation in adults and pediatric patients
Pharmacology
- Blocks dopamine receptors
- Blocks serotonin receptors in the chemoreceptor trigger zone of the CNS
- Enhances the response to acetylcholine of tissue in the upper GI tract
- Accelerates gastric emptying without stimulating gastric, biliary, or pancreatic secretions
- Increases lower esophageal sphincter tone
Dosage & Administration
- Recommended dosage for adults under 65 years of age is 1 spray (15 mg) in one nostril, 30 minutes before each meal and at bedtime (maximum of 4 sprays daily) for 2 to 8 weeks
- For gastroesophageal reflux, administer 10 to 15 mg, 30 minutes before each meal and at bedtime (maximum of 60 mg per day) for 4 to 12 weeks
- For diabetic gastroparesis, administer 10 mg, 30 minutes before each meal and at bedtime (maximum of 40 mg per day) for 2 to 8 weeks
- Dosage for prevention of nausea and vomiting associated with emetogenic cancer chemotherapy and postoperative nausea and vomiting varies based on the emetogenicity of the drugs and the patient's condition
Contraindications
- History of tardive dyskinesia or dystonic reaction to metoclopramide
- Stimulation of gastrointestinal motility might be dangerous such as in the presence of gastrointestinal hemorrhage, mechanical obstruction, or perforation
- Patients with pheochromocytoma or other catecholamine-releasing paragangliomas
- Patients with epilepsy
- Hypersensitivity to metoclopramide

Precautions & Warnings
- Avoid concomitant use of other drugs known to cause Tardive Dyskinesia (TD), Other Extrapyramidal Symptoms (EPS), and Neuroleptic Malignant Syndrome (NMS)
- Avoid use in patients with Parkinson’s Disease
- Discontinue if symptoms occur and seek immediate medical attention
- Avoid use in cases of depression and suicidal ideation/suicide
