Dosage

• 40 mg daily dosage for adults
• 80-120 mg for resistant edema
• 1 - 3 mg/kg body weight daily for children
• 20-40 mg daily for elderly

Indications

• Fluid retention associated with chronic congestive cardiac failure
• Fluid retention associated with acute congestive cardiac failure
• Fluid retention associated with chronic renal failure
• Maintenance of fluid excretion in acute renal failure
• Hypertension

Mode of Action

• Monosulphonyl diuretic
• Inhibition of sodium and chloride transport in the ascending limb of the loop of Henle
• Prompt onset of action

Pharmacology

• Effective diuretic even in low glomerular filtration rate (GFR)
• Rapidly excreted in the urine

Interaction

• Potential blood pressure fall with ACE inhibitors
• Increased serum lithium levels
• Potentiating toxic effects of nephrotoxic antibiotics

Contraindications

• Anuria
• Electrolyte deficiency
• Pre-comatose states associated with liver cirrhosis
• Hypersensitivity to furosemide or sulphonamides

Side Effects

• Disturbed electrolytes and water balance
• Alkalosis
• Uric acid retention
• Hyperglycemia
• Glycosuria

Pregnancy & Lactation

• Category C
• Cautious use in cardiogenic shock and first trimester of pregnancy
• Caution in nursing mothers

Precautions & Warnings

• Increased risk of acute retention in patients with prostatic hypertrophy or micturition impairment
• Potential blood pressure fall with ACE inhibitors
• Increased toxicity with nephrotoxic antibiotics
• Impaired ability for hazardous tasks

Overdose Effects

• Dependent on electrolyte and fluid loss
• Clinical consequences such as hypovolaemia, dehydration, cardiac arrhythmias
• No specific antidote known
