Indications

- Treatment of locally advanced or metastatic NSCLC after failure of prior chemotherapy
- First-line treatment of locally advanced, unresectable or metastatic pancreatic cancer in combination with gemcitabine
Description
- Kinase inhibitor with chemical name N-(3-ethynylphenyl)-6,7-bis(2-methoxyethoxy)4-quinazolinamine
- Reversibly inhibits the kinase activity of EGFR
- Higher binding affinity for EGFR exon 19 deletion or exon 21 (L858R) mutations than wild-type receptor
- Inhibition of other tyrosine kinase receptors not fully characterized
Dosage and Administration
- Recommended daily dose for NSCLC: 150 mg on an empty stomach
- Recommended daily dose for pancreatic cancer: 100 mg in combination with gemcitabine on an empty stomach
- Dose reduction for severe reactions with CYP3A4 inhibitors
- Dose increase for concomitant use with CYP3A4 inducers
- Dose adjustment for concurrent cigarette smoking
Interactions
- Interaction with coumarin-derived anticoagulants leading to increased INR and bleeding adverse reactions
- Metabolized predominantly by CYP3A4
- Co-administration with CYP3A4 inhibitors or inducers
- Co-administration with drugs affecting gastric pH
Side Effects
- Common adverse reactions: rash, diarrhea, anorexia, fatigue, dyspnea, cough, nausea, vomiting
- Serious adverse reactions include ILD, renal failure, hepatotoxicity, gastrointestinal perforation, skin disorders, myocardial infarction, cerebrovascular accident, ocular disorders, hemorrhage

Precautions and Warnings
- Occurrences of ILD, renal failure, hepatotoxicity, gastrointestinal perforations, and skin disorders

- No established safety and effectiveness in pediatric patients
- No overall differences in safety or efficacy in geriatric patients
Overdose Effects
- Tolerability of single oral doses up to 1,000 mg in healthy subjects
- Tolerability of weekly doses up to 1,600 mg in cancer patients
- Unacceptable severe adverse reactions above recommended dose
- Symptomatic treatment for suspected overdose
