Description
- Docetaxel is an antineoplastic agent that disrupts the microtubular network in cells, inhibiting mitosis and cellular functions
Indications
- Indicated for Breast Cancer (BC) as a single agent for locally advanced or metastatic BC after chemotherapy failure
- Indicated for Non-small Cell Lung Cancer (NSCLC) as a single agent for locally advanced or metastatic NSCLC after platinum therapy failure
- Indicated for Castration-Resistant Prostate Cancer (CRPC) with prednisone in metastatic CRPC
- Indicated for Gastric Adenocarcinoma (GC) with cisplatin and fluorouracil for untreated, advanced GC including the gastroesophageal junction
- Indicated for Squamous Cell Carcinoma of the Head and Neck (SCCHN) with cisplatin and fluorouracil for induction treatment of locally advanced SCCHN
Pharmacology
- Acts by disrupting the microtubular network in cells
- Promotes the assembly of tubulin into stable microtubules
- Inhibits disassembly of microtubules
- Binds to free tubulin, decreasing the critical intracellular concentration of tubulin
Dosage & Administration
- Administer intravenously over 1 hr every 3 weeks
- Dosage ranges for different indications
- PVC equipment not recommended, use only a 21 gauge needle to withdraw docetaxel from the vial
- Premedicate with oral corticosteroids
- Adjust dose as needed
Interaction
- CYP3A4 substrate, metabolism may be modified by concomitant administration of certain compounds
Contraindications
- Neutrophil counts of <1500 cells/mm3
- History of severe hypersensitivity reactions to docetaxel or drugs formulated with polysorbate 80
Side Effects
- Common adverse reactions include infections, neutropenia, anemia, febrile neutropenia, hypersensitivity, thrombocytopenia, neuropathy, dysgeusia, dyspnea, constipation, anorexia, nail disorders, fluid retention, asthenia, pain, nausea, diarrhea, vomiting, mucositis, alopecia, skin reactions, myalgia
- Incidence varies depending on the indication
Pregnancy & Lactation
- Can cause fetal harm when administered to a pregnant woman
- No information regarding presence in human milk or effects on breastfed child
Precautions & Warnings
- Administered under supervision of a physician experienced in antineoplastic agents
- Higher risk of severe adverse reactions in patients with hepatic impairment
- Not to be given to patients with neutrophil counts of less than 1,500 cells/mm3
- Fatal cases of enterocolitis reported, close monitoring required
- Severe hypersensitivity reactions may occur, close monitoring required
- Treatment related acute myeloid leukemia may occur
Overdose Effects
- No known antidote for docetaxel overdose
- Complications may include bone marrow suppression, peripheral neurotoxicity, mucositis
- Patients should receive therapeutic G-CSF as soon as possible after discovery of overdose
