Indications
- Palliative treatment of advanced carcinoma of the breast or endometrium
- Treatment of anorexia, cachexia, or unexplained weight loss in AIDS & cancer patients
Pharmacology
- Synthetic, antineoplastic and progestational nature
- Unknown precise mechanism of action against endometrial carcinoma
- Inhibition of pituitary gonadotrophin production
- Decrease in estrogen secretion
- Direct cytotoxic effect on tumor cells
- Alters the action of other steroid hormones
- Modifies and abolishes the stimulatory effects of estrogen on cells
- Estimates of plasma levels-dependent on measurement method
- Peak plasma concentrations occur 2 to 3 hours after a single oral dose
- Plasma half-life of 33 to 38 hours
- Approximately 66% of administered dose excreted in urine
- Approximately 20% of administered dose excreted in feces
Dosage & Administration
- Tablet dosage for breast cancer: 160 mg/day
- Tablet dosage for endometrial carcinoma: 40-320 mg/day in divided doses
- Recommended adult initial dosage of Oral Suspension: 800 mg/day (20 ml/day)
Interaction
- No investigated interactions with concomitant medications
- Estrogens and Progestogens may cause fluid retention
Contraindications
- History of hypersensitivity to Megestrol Acetate or any component
- Known or suspected pregnancy
Side Effects
- Frequent weight gain associated with increased appetite
- Thromboembolic phenomena including thrombophlebitis and pulmonary embolism
- Uncertain glucocorticoid effects
- Nausea, dyspnea, tumor flare, hyperglycemia, glucose intolerance, alopecia, hypertension, carpal tunnel syndrome, mood changes, hot flashes, malaise, asthenia, lethargy, sweating, rash
Pregnancy & Lactation
- Pregnancy Category D
- Not recommended for use during the first four months of pregnancy
- Very small amounts excreted in mother's milk
- Potential for adverse effects on newborn
- Nursing should be discontinued during treatment
Precautions & Warnings
- Close surveillance for patients treated for recurrent or metastatic cancer
- Caution in patients with history of thromboembolic disease
- Exacerbation of preexisting diabetes with increased insulin requirements
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- Insufficient data for patients 65 years and older
Overdose Effects
- No serious unexpected side effects from dosages as high as 1600 mg/day
