Indications
- Reduces urate/uric acid formation
- Manages gouty arthritis
- Reduces skin tophi
- Manages nephrolithiasis
- Manages 2,8-dihydroxyadenine (2,8-DHA) renal stones
- Reduces recurrence of mixed calcium oxalate renal stones
Pharmacology
- Xanthine oxidase inhibitor
- Reduces production of uric acid
- Structural analogue of hypoxanthine
- Inhibits xanthine oxidase enzyme
- Approximately 90% absorbed from GI tract
- Peak plasma levels at 1.5 to 4.5 hours
Dosage & Administration
- Adults:
- 100 to 200 mg daily in mild conditions
- 300 to 600 mg daily in moderately severe conditions
- 700 to 900 mg daily in severe conditions
- Children: 10 to 20 mg/kg body weight/day
- Elderly: Use lowest effective dosage
- Renal impairment: Adjust dosage based on severity
Interaction
- Reduces activity of 6-mercaptopurine or azathioprine
- May increase plasma half-life of vidarabine
- Requires monitoring of theophylline levels
- Enhanced toxic effects when used with certain drugs
- May increase plasma concentration of ciclosporin
Contraindications
- Hypersensitivity to allopurinol

Precautions & Warnings

- Use with care in patients with hypertension or cardiac insufficiency
Overdose Effects
- Ingestion reported up to 22.5 g without adverse effects
- Symptoms including nausea, vomiting, and dizziness
- Adequate hydration for optimum diuresis
- Haemodialysis if necessary
