Description

  • Temozar is a medication indicated for the treatment of adult patients with newly diagnosed glioblastoma multiforme concomitantly with radiotherapy and then as maintenance treatment, and for the treatment of adult patients with refractory anaplastic astrocytoma.
  • Temozar is not directly active but undergoes rapid nonenzymatic conversion at physiologic pH to the reactive compound Methyl Triazen Imidazole Carboxamide (MTIC), with cytotoxicity thought to be primarily due to alkylation of DNA.
  • Temozar should be taken on an empty stomach at least 1 hour before meals.
  • Temozar is contraindicated in patients with a history of hypersensitivity reaction to any of its components, and also in patients with a history of hypersensitivity to DTIC.
  • The most common adverse effects of Temozar are alopecia, fatigue, nausea, vomiting, headache, constipation, anorexia, convulsions, rash, hemiparesis, diarrhea, asthenia, fever, dizziness, coordination abnormality, viral infection, amnesia, insomnia, and Grade 3 to 4 hematologic laboratory abnormalities such as lymphopenia, thrombocytopenia, neutropenia, and leukopenia, as well as allergic reactions.
  • Temozar is classified under the therapeutic class of Cytotoxic Chemotherapy, and should be stored at a temperature not exceeding 25°C in a dry place, protected from light and moisture, and should not be frozen.

    • 75 mg/m^2 for 42 days concomitant with focal radiotherapy
    • Initial maintenance dose of 150 mg/m^2 once daily for Days 1-5 of a 28-day cycle for 6 cycles
    • Initial dose of 150 mg/m^2 once daily for 5 consecutive days per 28-day treatment cycle
  • Administration:
    • Take on an empty stomach
    • Take at least 1 hour before meals
  • Interaction:
    • Administration of valproic acid decreases oral clearance of Temozar by about 5%
  • Contraindications:
    • Hypersensitivity reaction to any components
    • Hypersensitivity to DTIC

    • Fetal harm can occur when administered to a pregnant woman
  • Use in Special Populations:
    • Safety and effectiveness in pediatric patients have not been established
    • Caution in patients with severe renal impairment
    • Caution in patients with severe hepatic impairment
  • Overdose Effects:
    • Dose-limiting toxicity
    • Adverse reactions including marrow suppression, infections, and death reported in cases of overdose
