Pharmacology
Furosemide is a monosulphonyl diuretic with distinctive action on renal tubular function, including prompt onset of action, inhibition of sodium and chloride transport in the ascending limb of the loop of Henle, and independence of action from acid-base balance changes.
Dosage & Administration
- Furosemide 40 mg Tablet:
- Edema Adult: Initial adult dose of 40 mg daily, reduced to 20 mg daily or 40mg on alternate days, with some patients requiring daily doses of 80mg or higher in divided doses
- Edema Children: Doses based on age and weight, ranging from neonate to 12-18 years
- Hypertension Adult: Usual initial dose of 80 mg, usually divided into 40 mg twice a day, with dosage adjustment according to response
- Hypertension Children: Dosage based on body weight, up to a maximum dose of 40mg/day
- Furosemide 250 mg Tablet: For selected patients with advanced chronic renal failure, diuretic therapy may be started with a 250 mg orally, with a maximum daily dose of 1000 mg
- Furosemide Syrup: Wide therapeutic range of dosage, with usual initial daily dose of 40 mg and maximum daily dose of 1500 mg
- Furosemide Injection:
- Edema Adult: Intramuscular or intravenous doses of 20-50 mg initially, with larger doses recommended to be given by slow intravenous infusion for a maximum daily dose of 1500 mg
- Edema Children: Parenteral doses based on age and weight
- Hypertension: Doses based on age and body weight, with dosage adjustment for elderly patients
Interaction
- Potential interaction with ACE inhibitors, serum lithium levels, and nephrotoxic antibiotics
Contraindications
- Contraindicated in anuria, electrolyte deficiency and pre-comatose states associated with liver cirrhosis
- Hypersensitivity to furosemide or sulphonamides
Side Effects
- Disturbance in electrolytes and water balance
- Prolonged use may produce alkalosis
- Potential for uric acid retention and acute gout
- Potential for hyperglycemia and glycosuria
Pregnancy & Lactation
Category C drug with cautious use in cardiogenic shock complicated by pulmonary oedema and in the first trimester of pregnancy. Caution required in nursing mothers.
Precautions & Warnings
- Increased risk for patients with prostatic hypertrophy or impairment of micturition
- Potential marked fall in blood pressure when ACE inhibitors are added to therapy
- Potential increased toxic effects of nephrotoxic antibiotics
Overdose Effects
- Signs and symptoms: Clinical picture depends primarily on the extent and consequences of electrolyte and fluid loss, e.g. hypovolaemia, dehydration, cardiac arrhythmias
- Management: No specific antidote known, with attempts to limit further systemic absorption through measures such as gastric lavage or activated charcoal
