Description
- Indicated for fluid retention associated with congestive cardiac failure
- Effective in chronic renal failure
- Rapid onset of action
- Absorbed from gastrointestinal tract
- Marked fall in blood pressure when ACE inhibitors added
Dosage & Administration
- Initial adult dose is 40 mg daily
- For hypertension, the initial dose is 80 mg divided into 40 mg twice a day
- Dosage should be titrated until required response is achieved
- Furosemide may be carefully titrated up to 600 mg/day for severe cases
Interactions
- Interaction with ACE inhibitors may cause marked fall in blood pressure
- Concomitant administration of potent diuretics can increase the toxic effects of nephrotoxic antibiotics
Contraindications
- Contraindicated in anuria
- Contraindicated in electrolyte deficiency
- Contraindicated in pre-comatose states associated with liver cirrhosis
Side Effects
- Electrolytes and water balance may be disturbed
- Prolonged use can produce alkalosis
- May provoke hyperglycemia and glycosuria
Pregnancy & Lactation
- Caution should be observed in the first trimester of pregnancy
- Furosemide may inhibit lactation or pass into breast milk
Precautions & Warnings
- Increased risk of developing acute retention in patients with prostatic hypertrophy
- Special caution should be taken when driving or operating machinery
Overdose Effects
- Clinical picture in overdose depends primarily on electrolyte and fluid loss
- No specific antidote is known
