Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhancement of central noradrenergic and serotonergic activity
  • Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Potent antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Antagonist at muscarinic receptors
  • Sedative effects due to histamine (H<sub>1</sub>) receptor antagonism
  • Metabolism and elimination via urine and faeces

Dosage & Administration

  • Starting dose of 15 mg/day
  • Administration in single dose, preferably in the evening or prior to sleep
  • Effective dose range of 15 to 45 mg/day
  • Elimination half-life of approximately 20 to 40 hours
  • Recommended dose changes at intervals of less than 1 to 2 weeks

Interaction

  • Clinically significant interactions with Monoamine Oxidase Inhibitors (MAOI) & other serotonergic drugs
  • Interrupts the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
  • Patient should avoid Alcohol & Diazepam

Contraindications

  • Contraindicated in patients with known hypersensitivity to Mirtazapine or its excipients
  • Concomitant use with monoamine oxidase (MAO) inhibitor is contraindicated
  • Not to be used within 14 days of initiating or discontinuing therapy with a MAOI

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify physician if they become pregnant or intend to become pregnant during therapy
  • Patients should notify physician if they are breastfeeding an infant

Precautions & Warnings

  • Alertness to emergence of various behavioral changes early in treatment
  • Risk of developing agranulocytosis
  • Impairment of judgment, thinking, and motor skills
  • Potential for clinically significant ALT (SGPT) elevations
