Indications
- Fluid retention associated with chronic congestive cardiac failure

Dosage & Administration
- Initial adult dose of 40mg daily
- Reduced to 20mg daily or 40mg on alternate days
- Daily doses of 80mg or higher may be required
- Dose titration up to 600mg/day

- Starting dose of 250mg for selected patients with advanced chronic renal failure

Interaction
- Marked fall in blood pressure when ACE inhibitors are added
- Increased serum lithium levels
- Enhanced toxic effects of nephrotoxic antibiotics
Contraindications
- Contraindicated in anuria
- Electrolyte deficiency
- Pre-comatose states associated with liver cirrhosis
- Hypersensitivity to furosemide or sulphonamides
Side Effects
- Disturbed electrolytes and water balance
- Alkalosis
- Uric acid retention
- Provocation of hyperglycemia and glycosuria
Pregnancy & Lactation
- Pregnancy category C
- Caution in cardiogenic shock and first trimester of pregnancy
- Monitoring of blood pressure and pulse during rapid diuresis
- Caution in nursing mothers
Precautions & Warnings
- Increased risk of acute retention in patients with prostatic hypertrophy
- Risk of undesirably pronounced fall in blood pressure
- Impairment of ability to concentrate and react
- Risk during driving or performing hazardous tasks
Overdose Effects
- Clinical picture depends on electrolyte and fluid loss
- Signs such as hypovolaemia, dehydration, cardiac arrhythmias
- Management includes limiting systemic absorption and gastric lavage
