Indications
- Treatment of men with erectile dysfunction
- Treatment of pulmonary arterial hypertension
Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation and inflow of blood into penile tissues
- Requires sexual stimulation for effectiveness
Pharmacology
- Mechanism of Action: Involves release of nitric oxide and activation of guanylate cyclase
- Pharmacokinetics and Metabolism: Rapid absorption, hepatic metabolism, active metabolite similar to parent compound
Dosage & Administration
- Recommended dose for erectile dysfunction: 50 mg taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg based on toleration
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism primarily mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors and inducers of these isoenzymes can affect clearance
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with organic nitrates

Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Evaluation of erectile dysfunction should include determination of potential underlying causes
- Caution advised when co-administered with alpha-blockers
- General caution in patients with bleeding disorders, active peptic ulceration, anatomical penile deformities, and conditions predisposing to priapism
Overdose Effects
- Adverse events similar to those seen at lower doses
- Supportive measures should be adopted as required
