Medicine Details

Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis
  • Swelling due to excess fluid retention
  • Hyperaldosteronism
  • Resistant edema associated with secondary hyperaldosteronism

Pharmacology

  • Complementary mechanisms and sites of action
  • Additive or synergistic diuretic
  • Inhibition of Na+/K+/2Cl- co-transporter
  • Blockage of reabsorption of sodium, potassium, and chloride ions
  • Induced potassium loss
  • Inhibition of reabsorption of sodium in exchange for potassium
  • Antagonizing the action of aldosterone

Dosage & Administration

  • Dosage range of Furosemide 20 and spironolactone 50 mg
  • Dosage range of Furosemide 40 and spironolactone 50 mg
  • Not suitable for use in children
  • Slower excretion in the elderly

Interaction

  • Increased risk of hyperkalemia with ACE inhibitors or potassium salts
  • Elevated levels of cardiac glycosides such as digoxin in the blood
  • Potential digitalis toxicity
  • Possible hypokalemia when used with corticosteroids
  • Reduction of blood pressure lowering and diuretic effects when used with indomethacin and other NSAIDs
  • Increased ototoxicity of aminoglycoside antibiotics in combination with Furosemide
  • Reduced natriuretic and anti-hypertensive effect in simultaneous administration with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function
  • Hyperkalaemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

  • Transient increase in blood-urea-nitrogen concentrations
  • Mild acidosis
  • Hyponatremia
  • Excessive diuresis leading to dehydration
  • Reduction in blood volume with circulatory collapse
  • Serious depletion of potassium and magnesium leading to cardiac arrhythmias

Pregnancy & Lactation

  • Crossing the placental barrier
  • Animal teratology studies indicating possible fetal abnormalities
  • Requirement for appropriate contraceptive measures in women of childbearing age
  • Detection of metabolites in breast milk
  • Discontinuation of breast-feeding if treatment is essential

Precautions & Warnings

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes
  • Enlarged prostate
  • Hypotension
  • Hypovolemia
