Indications
- Treatment of nausea and vomiting of pregnancy
- For women who do not respond to conservative management
Pharmacology
- Doxylamine Succinate is an antihistamine
- Blocks Histamine (H1) receptor
- Crosses the blood brain barrier
- High affinity for H1 receptors in the brain
- Decreases the action of histamine at the H1 receptor
- Inhibits both vestibular system & Muscarinic receptor
- Affects the vestibular system
- Decreases the stimulation of the vomiting center
- Pyridoxine Hydrochloride is a vitamin B6 analog
- Used to prevent nausea and vomiting
- Antiemetic properties
Dosage & Administration
- Take one tablet orally at bedtime on Day 1
- If symptoms persist, increase the daily dose on Day 2
- Maximum recommended dose is two tablets per day
- Take on an empty stomach with a glass of water
- Swallow tablets whole
- Do not crush, chew, or split the tablets
- Take daily and not on an as needed basis
Interaction
- Contraindicated with monoamine oxidase inhibitors (MAOIs)
- Not recommended with alcohol and other CNS depressants
Contraindications
- Known hypersensitivity to Doxylamine Succinate or Pyridoxine Hydrochloride
- Concurrent use of monoamine oxidase (MAO) inhibitors
- Monoamine oxidase inhibitors intensify and prolong adverse effects
Side Effects
- Somnolence
- Accidents resulting from CNS depressants
Pregnancy & Lactation
- Pregnancy category A
- Excreted into breast milk
- Caution should be exercised while breastfeeding
Precautions & Warnings
- May cause somnolence
- Avoid activities such as driving or operating heavy machinery
- Not recommended with concurrent use of CNS depressants including alcohol
- Use with caution in women with asthma, increased intraocular pressure, narrow angle glaucoma, stenosing peptic ulcer, pyloroduodenal obstruction or urinary bladder-neck obstruction
Overdose Effects
- Signs and symptoms of intoxication may not be apparent immediately
- Signs and symptoms may include restlessness, dryness of mouth, dilated pupils, sleepiness, vertigo, mental confusion, tachycardia
- At toxic doses, exhibits anticholinergic effects including seizures, rhabdomyolysis, acute renal failure, death
- Treatment consists of gastric lavage or activated charcoal, whole bowel irrigation, symptomatic treatment
