Indications
- Treatment of erectile dysfunction

Description
- Selectively reverses cGMP-specific PDE5 enzyme
- Causes smooth muscle relaxation and inflow of blood into penile tissues
- Requires sexual stimulation to be effective
Pharmacology
- Mechanism of Action involves release of nitric oxide during sexual stimulation
- Activates guanylate cyclase, leading to increased cGMP levels and smooth muscle relaxation
- Absorbed rapidly after oral administration
- Predominantly cleared by hepatic metabolism
- Metabolized by cytochrome P450 3A4
Dosage & Administration
- Recommended starting dose is 50 mg, to be taken 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Maximum dosing frequency is once per day
Interaction
- Metabolism mediated by CYP3A4 and CYP2C9 isoforms
- Inhibitors of CYP3A4 reduce clearance, while inducers increase clearance
- Potent CYP3A4 inhibitors can significantly increase systemic exposure
- Concomitant use with certain medications may affect plasma levels
Contraindications
- Hypersensitivity to any component
- Concurrent use with organic nitrates
Side Effects
- Body as a whole: face edema, chills, abdominal pain
- Cardiovascular: angina pectoris, tachycardia, hypotension
- Digestive: vomiting, dysphagia, liver function tests abnormal
- Hemic and Lymphatic: anemia and leukopenia
- Metabolic and Nutritional: hyperglycemia, hyperuricemia, hypoglycemic reaction
- Musculoskeletal: myalgia, bone pain, synovitis
- Nervous: paresthesia, vertigo, insomnia
- Respiratory: dyspnea, cough increased
- Skin and Appendages: urticaria, skin ulcer, pruritus
- Special Senses: sudden decrease or loss of hearing, photophobia
- Urogenital: cystitis, abnormal ejaculation, genital edema
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Evaluation of erectile dysfunction should include determination of potential underlying causes
- Caution advised when PDE5 inhibitors are co-administered with alpha-blockers
- Consideration should be given to patient stability on alpha-blocker therapy
- Potential cardiac risk in patients with preexisting cardiovascular disease
- Caution in patients with anatomical deformation of the penis or conditions predisposing to priapism
- Potential for sensitivity to vasodilators in certain patient groups
Overdose Effects
- Incidence rates and severities of adverse events are increased at higher doses
- Standard supportive measures are required in case of overdose
