Indications

Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
Mechanism of Action
- Inhibition of PDE5 by Activa produces increased levels of cGMP in the corpus cavernosum
- Results in smooth muscle relaxation and inflow of blood into the penile tissues, thereby producing an erection
Pharmacokinetics and Metabolism
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Terminal half lives of about 4 hours
Absorption and Distribution
- Maximum observed plasma concentrations are reached within 30 to 120 minutes of oral dosing
- Rate of absorption reduced when taken with high fat meal
Metabolism and Excretion
- Sildenafil is cleared by hepatic microsomal isoenzymes
- Excreted as metabolites predominantly in the feces
Pharmacokinetics in Special Populations
- Reduced clearance of Sildenafil in healthy elderly volunteers
- Approximately 84% and 107% higher plasma AUC values in elderly volunteers
Dosage & Administration
- Recommended dose is 50 mg taken 1 hour before sexual activity
- Dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg
Interaction
- Metabolism primarily mediated by the cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce Activa clearance and inducers may increase Activa clearance
Contraindications
- Contraindicated in patients with hypersensitivity to any component
- Contraindicated in patients using organic nitrates

- Not indicated for use by women
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Consideration given to patients with underlying conditions such as left ventricular outflow obstruction
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
