Indications
- Reduce rate of MI and stroke in patients with non-ST-segment elevation ACS
- Reduce rate of MI and stroke in patients with acute ST-elevation myocardial infarction
- Reduce rate of MI and stroke in patients with established peripheral arterial disease or recent MI or stroke
Pharmacology
Irreversibly binds to P2Y12 class of ADP receptors on platelets, inhibiting platelet activation and aggregation
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome, followed by 75 mg once daily
- 75 mg once daily for recent MI, stroke, or established peripheral arterial disease
- Orally with or without food
Interaction
- Interacts with NSAIDs, warfarin, SSRIs, SNRIs increasing risk of bleeding
- Avoid concomitant use with CYP2C19 inhibitors like omeprazole or esomeprazole
- Avoid concomitant use with Repaglinide as it increases plasma concentrations of Repaglinide
Contraindications
Hypersensitivity to the drug substance or any component, active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, diarrhea, gastrointestinal discomfort, hemorrhage, skin reactions
- Rare: Acquired hemophilia, anemia, angioedema, arthralgia, bone marrow disorders
Pregnancy & Lactation
Use during pregnancy only if clearly needed, unknown excretion in human breast milk, decision should consider importance of the drug to the mother
Precautions & Warnings
- Metabolism impaired by genetic variations in CYP2C19 and drugs that inhibit CYP2C19
- Inhibits platelet aggregation for lifetime of the platelet, increasing risk of bleeding
- Discontinuation increases risk of cardiovascular events, discontinue prior to major surgery, TTP reported requiring urgent treatment
- Hypersensitivity reactions reported in patients receiving clopidogrel or history of hypersensitivity to other thienopyridines
Use in Special Populations
- Safety and effectiveness in pediatric populations not established
- No dosage adjustment necessary in elderly patients
Overdose Effects
May lead to bleeding complications, platelet transfusion may restore clotting ability
