Indications

  • Management of edema of cardiac, renal and hepatic origin
  • Management of hypertension
  • Sole therapeutic agent or in combination with other antihypertensive agents

Pharmacology

  • Inhibition of Na+/K+/2Cl- carrier system
  • Decrease in reabsorption of sodium and chloride
  • Increase in delivery of tubular fluid and electrolytes
  • Increase in aldosterone production
  • Increase in urinary excretion of sodium, chloride, and water
  • Reduction in plasma volume and blood pressure

Dosage & Administration

  • Initial oral dose of 10 mg or 20 mg for congestive heart failure
  • Initial oral dose of 20 mg for chronic renal failure
  • Initial oral dose of 5 mg or 10 mg for hepatic cirrhosis
  • Usual initial oral dose of 2.5-5 mg for hypertension

Interaction

  • Increased risk of severe hypokalaemia with certain medications
  • Increased risk of lithium toxicity
  • Potential for ototoxicity and nephrotoxicity with specific medications
  • Increased risk of toxicity with digoxin
  • Reduced diuretic effect with NSAIDs
  • Increased risk of hypotension with antihypertensives

Contraindications

  • Hypersensitivity to torasemide and other sulfonyl ureas
  • Anuric patients

Overdose Effects

  • Signs and symptoms include dehydration, hypovolemia, hypotension, and hypokalemia
  • Treatment should consist of fluid and electrolyte supplement
