Indications
- Post-menopausal osteoporosis
- Renal osteodystrophy
- Postsurgical hypoparathyroidism
- Idiopathic hypoparathyroidism
- Pseudohypoparathyroidism
- Secondary hyperparathyroidism in patients with moderate to severe chronic renal failure (pre-dialysis)
- Vitamin D dependent rickets
Pharmacology
- Active metabolite of vitamin D3
- Promotes intestinal absorption of calcium
- Regulates bone mineralization
- Stimulates osteoblastic activity in the skeleton
Dosage & Administration
- Careful determination for each patient
- Recommended dose of 0.25 mcg twice daily for post-menopausal osteoporosis
- Initial dose of 0.25 mcg for renal osteodystrophy
- Initial dosage of 0.25 mcg/day for hypoparathyroidism and rickets
- Initial intravenous dose of 1-2 mcg administered three times weekly
Interactions
- Increased risk of hypercalcemia with thiazide diuretics
- Caution with digitalis treatment
- Hypermagnesemia with magnesium-containing drugs
Contraindications
- Hypersensitivity to any of its ingredients
- Diseases associated with hypercalcemia
Side Effects
- Early: weakness, headache, somnolence, nausea, vomiting
- Late: polyuria, anorexia, weight loss, nocturia, hyperthermia
- Infrequent: overt psychosis, mild pain on injection
Pregnancy & Lactation
- Potential risk to the fetus
- Monitoring of serum calcium levels for mother and infant
Precautions & Warnings
- Induces hypercalcemia and hypercalciuria at excessive dosage
- Caution in patients on digitalis
- Appropriate titration downward to a maintenance dose is recommended
Use in Special Populations
- Recommended dosage for adults, pediatric patients, and elderly
- Different initial dosage for pediatric patients less than 3 years of age
Overdose Effects
- Hypercalcemia, hypercalciuria, hyperphosphatemia
- General treatment involves discontinuation of therapy, low calcium diet
