Indications
- Control of hyperglycaemia
- Treatment of non-insulin-dependent diabetes mellitus (NIDDM type II)
- Adjunct to diet
- Reduction in daily dose of insulin
Pharmacology
- Stimulates insulin release from pancreas
- Increases insulin sensitivity
- Decreases hepatic glucose production
Dosage & Administration
- Adjustable dosage
- Should be given before a meal
- Initial dose of 5mg
- Dosage adjustments based on blood glucose response
- Maximum recommended daily dose of 15mg
- Maintenance dose ranging from 2.5 to 30mg daily
- Conservative dosing in elderly or debilitated patients
- Safe use with insulin
- Safe transition from other oral hypoglycaemic agents
- Pediatric use not established
Interaction
- Potentiation of hypoglycaemic action by certain drugs
- Observation for hypoglycaemia upon drug administration or withdrawal
- Possible hyperglycaemia with certain drugs
- Altered diabetic control with cyclophosphamide
Contraindications
- Hypersensitivity to Glipizide
- Juvenile onset diabetes
- Severe or unstable 'brittle' diabetes
- Diabetes complicated by ketosis and acidosis, major surgery, severe sepsis or severe trauma
- Severe renal, hepatic, or thyroid impairment
- Co-existent renal and hepatic disease
Side Effects
- Dose-related gastrointestinal complaints
- Cholestatic jaundice
- Allergic skin reactions
- Leucopenia, agranulocytosis, thrombocytopenia, haemolytic anaemia, aplastic anaemia, pancytopenia
- Hepatic porphyria and disulphiram-like reactions
- Hyponatraemia and syndrome of inappropriate antidiuretic hormone (SIADH) secretion
- Dizziness, drowsiness, headache
Pregnancy & Lactation
- Use during pregnancy justified by benefit-risk assessment
- Discontinuation before expected delivery date
- Not recommended during lactation
Precautions & Warnings
- Capable of producing severe hypoglycaemia
- Proper patient selection, dosage, and instructions are important to avoid hypoglycaemic episodes
- Elevated blood levels in renal or hepatic insufficiency
- Increased risk of serious hypoglycaemic reactions in susceptible patients
- Slowed metabolism and excretion in patients with impaired renal and/or hepatic function
- Exposure to stress may lead to loss of control on blood glucose
- Effectiveness may decrease over time
