Indications

  • Reduction of total cholesterol and LDL cholesterol in familial hypercholesterolemia
  • Treatment of mixed dyslipidemia and hypertriglyceridemia
  • Reduction of cardiac ischaemic events in coronary artery disease patients
  • Cholesterol reduction in diabetes mellitus or renal transplantation patients

Pharmacology

  • Selectively inhibits HMG-CoA reductase
  • Lowers plasma cholesterol and lipoprotein levels
  • Increases hepatic LDL receptors for enhanced LDL uptake and catabolism

Absorption

  • Rapidly absorbed after oral administration
  • Maximum plasma concentrations within 1 to 2 hours
  • Increase in extent of absorption with dose increase

Distribution

  • Mean volume of distribution approximately 381 liters
  • 98% bound to plasma proteins
  • Likely to be secreted in human milk based on animal studies

Metabolism

  • Extensively metabolized to ortho- and parahydroxylated derivatives
  • Importance of metabolism by cytochrome P450 3A4
  • Ortho-hydroxy metabolite undergoes further glucuronidation

Excretion

  • Primarily eliminated in bile after hepatic and/or extra-hepatic metabolism
  • Mean plasma elimination half-life approximately 14 hours

Interaction

  • Increased risk of myopathy with concurrent administration of certain drugs
  • Effect on plasma concentrations when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives, and warfarin

  • Hypersensitivity to any component of the medication
  • Active liver disease or unexplained persistent elevations of serum transaminases
  • History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors
