Description
- Long-acting aldosterone antagonist
- Specific pharmacologic antagonist of aldosterone
- Competitive binding of receptors at aldosterone-dependent sodium-potassium exchange site
- Increases sodium and water excretion
- Retains potassium and magnesium
Pharmacology
- Acts as a diuretic
- Acts as an antihypertensive drug
- Binds to mineralocorticoid receptor to block aldosterone actions
- Enhances expression of Na+ K+ ATPase and Na+ channel
- Interacts with cytoplasmic mineralocorticoid receptor to retain sodium and excrete potassium in the kidneys
Dosage & Administration
- Edema dosage range from 25 to 200 mg daily
- Combined therapy with other diuretics for rapid diuresis
- Primary hyperaldosteronism dosage of 100 to 400 mg daily
- Essential hypertension dosage range from 50 to 100 mg daily
- Hypokalemia dosage range from 25 to 100 mg daily
Interaction
- Severe hyperkalemia with ACE inhibitors
- Potentiation of orthostatic hypotension with alcohol, barbiturates, or narcotics
- Intensified electrolyte depletion with corticosteroids, ACTH
- Reduced renal clearance of lithium and increased risk of lithium toxicity with lithium
- Increased half-life of digoxin
Contraindications
- Acute renal insufficiency
- Significant impairment of renal function
- Anuria
- Hyperkalemia
- Sensitivity to Spironolactone
