Indications
- Erectile Dysfunction (ED)

Pharmacology
Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor. Inhibition of PDE5 increases cGMP in smooth muscle cells, causing smooth muscle relaxation and increased blood flow into the corpus cavernosum, resulting in penile erection. It also relaxes smooth muscles in the prostate and bladder, improving blood flow and urinary tract function.
Dosage & Administration
- For most patients the recommended starting dose for Erectile Dysfunction is 10 mg, with a maximum dosing frequency of once daily. Effective for up to 36 hours.
- The recommended dose for Benign Prostatic Hyperplasia is 5 mg taken at the same time every day.
- The recommended dose for Combined Erectile Dysfunction and Benign Prostatic Hyperplasia is 5 mg at the same time every day.
Interaction
May interact with Nitrates, Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids, Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, other HIV protease inhibitors, Rifampin, Carbamazepine, Phenytoin, and Phenobarbital.
Contraindications
- Use of Nitrates may increase hypotensive effects
- Hypersensitivity reactions to Tadalafil
Side Effects
- Common side effects include Headache, Dyspepsia, Back pain, Myalgia, Nasal pharyngitis, and Nasal congestion
- Rare occurrences of Change in Color Vision, Sudden vision loss, Hearing loss, Stevens-Johnson Syndrome, Exfoliative dermatitis, Angina, Stroke, Myocardial infarction, Severe hypotension, and Tachycardia
Pregnancy & Lactation
Tadalafil has been assigned to pregnancy category B by the USFDA and is only recommended for use during pregnancy when benefit outweighs risk. Caution should be used when administering tadalafil to nursing women.
Precautions & Warnings
Precaution should be taken in conditions such as Angina, renal impairment, hepatic impairment, bleeding concomitant with Nitrates, Alpha Blockers, Alcohol, CYP3A4 Inhibitors, and other PDE5 inhibitors.
