Indications

  • Congestive heart failure
  • Hepatic cirrhosis with ascites and oedema
  • Nephrotic syndrome
  • Primary hyperaldosteronism
  • Essential hypertension
  • Treatment of patients with hypokalemia

Description

  • Long-acting aldosterone antagonist
  • Specific pharmacologic antagonist of aldosterone
  • Competitive binding of receptors at aldosterone dependent sodium-potassium exchange site
  • Causes increased excretion of sodium and water
  • Retention of potassium and magnesium

Pharmacology

  • Acts as a diuretic and antihypertensive drug
  • Interacts with a cytoplasmic mineralocorticoid receptor
  • Enhances the expression of Na+ K+ ATPase and Na+ channel
  • Blocks the actions of aldosterone on gene expression
  • Hormonal function is to retain sodium and excrete potassium in the kidneys

Dosage & Administration

  • Initial daily dosage of 100 mg
  • May range from 25 to 200 mg daily
  • Combined therapy with other diuretics
  • Doses of 100 to 400 mg daily in hyperaldosteronism
  • Initial daily dosage of 50 to 100 mg for essential hypertension
  • Ranging from 25 mg to 100 mg daily for hypokalemia

Interaction

  • Concomitant administration with ACE inhibitors may cause severe hyperkalemia
  • Potentiation of orthostatic hypotension with alcohol, barbiturates, or narcotics
  • Intensified electrolyte depletion with corticosteroids or ACTH
  • Avoid concomitant use with lithium due to increased risk of toxicity
  • Increases the half-life of digoxin

Contraindications

  • Acute renal insufficiency
  • Significant impairment of renal function
  • Anuria
  • Hyperkalaemia
  • Sensitivity to Spironolactone

Side Effects

  • Gynaecomastia
  • GI symptoms including cramping and diarrhoea
  • Drowsiness
  • Lethargy
  • Headache
  • Urticaria
  • Mental confusion
  • Impotence
  • Irregular menses or amenorrhoea
  • Post-menopausal bleeding

Pregnancy & Lactation

  • Not recommended during pregnancy
  • Active metabolite appears in breast milk

Precautions & Warnings

  • Observation for evidence of fluid or electrolyte imbalance
  • Risk of hyperkalemia in patients with impaired renal function or excessive potassium intake

Overdose Effects

  • Drowsiness
  • Mental confusion
  • Dizziness
  • Diarrhea
  • Vomiting
  • Induce vomiting or stomach lavage in case of overdose
