Indications

  • Treatment of moderate to severe painful conditions
  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

  • Centrally acting synthetic analgesic compound
  • Inhibits reuptake of neurotransmitters - serotonin and noradrenaline
  • Modifies transmission of pain impulses
  • Activates descending serotonergic pathways
  • Stimulation of mu-opioid receptors
  • Modulation of central monoaminergic inhibitory pathways

Dosage & Administration

  • Usual doses of 50 to 100 mg every 4-6 hours
  • Initial dose of 100 mg for acute pain
  • Initial dose of 50 mg for chronic painful conditions
  • Total daily dosage by mouth should not exceed 400 mg
  • Sustained release capsule or tablet every 12 hours
  • Injection dose of 50-100 mg every 4-6 hours
  • Suppository dose of 100 mg Tramadol Hydrochloride 6 hourly

Interaction

  • No concern for drug interactions except with MAO inhibitors
  • Concomitant administration of carbamazepine increases Trado metabolism

Contraindications

  • Hypersensitivity to Tramadol
  • Acute intoxication with alcohol, hypnotics, opioids or psychotropic drugs

Pregnancy & Lactation

  • Crosses the placenta
  • Use during pregnancy only if potential benefit justifies risk to the fetus
  • Not to be used during breastfeeding

Precautions & Warnings

  • Respiratory depression risk
  • Not recommended for opioid-dependent patients
  • Caution with concomitant CNS depressants
  • Caution with concomitant MAO inhibitors
  • Caution in patients with increased intracranial pressure or head injury

Use in Special Populations

  • Can be given to children from age 1 at 1-2 mg/kg body weight
  • Suppository not to be administered in children and adolescents below the age of 14
