Indications
- Adjunct to diet & exercise for type 2 diabetes mellitus
Pharmacology
- Inhibition of DPP-4 enzyme
- Increase in insulin secretion
- Decrease in glucagon secretion
- Reduced hepatic glucose production
Dosage & Administration
- Initiation based on current Metformin dose
- Twice daily intake
- Morning and evening dosage
- Administration with food to reduce gastrointestinal symptoms
Interaction
- No clinically relevant pharmacokinetic interactions observed
- Low potential for drug interactions
- No interaction with certain oral antidiabetics
- Increased Cmax and AUC with certain medications
Contraindications
- Hypersensitivity to Vildagliptin or Metformin Hydrochloride
- Renal disease or dysfunction
- Acute myocardial infarction
- Septicaemia
- Congestive heart failure
- Metabolic acidosis including diabetic ketoacidosis

- Tremor
- Dizziness
- Nausea
- Hypoglycaemia
Pregnancy & Lactation
- Not recommended during pregnancy
- No studies in pregnant women
- Not to be administered to breastfeeding women
Precautions & Warnings
- Risk of lactic acidosis
- Monitoring of serum creatinine in patients with renal function
- Special caution in elderly patients
- Monitoring of Liver Function Tests
- Discontinuation 48 hours before elective surgery
- Not recommended in pediatric patients
- Monitoring of renal function in geriatric patients
- Not for use in patients with renal or hepatic impairment
Use in Special Populations
- Not recommended for use in children below 18 years of age
- Monitoring of renal function in elderly patients
