Description
- Exerts direct inhibitory effect on the parasympathetic nervous system
- Relaxing effect on smooth musculature
Indications
- Adjunct in the treatment of all forms of parkinsonism
- Useful as adjuvant therapy with levodopa
- Control of extrapyramidal disorders caused by CNS drugs
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for M1 subtype receptors
- Higher affinity for central muscarinic receptors in cerebral cortex
- Potential modification of nicotinic acetylcholine receptor neurotransmission
- Indirect enhancement of dopamine release in the striatum
Dosage & Administration
- Initial dose should be low and increased gradually
- Dosage individualized, especially in patients over 60 years
- May be given before or after meals based on patient's reaction
- Abrupt withdrawal may result in acute exacerbation of Parkinsonism symptoms
- Total daily dose for idiopathic Parkinsonism and drug-induced Parkinsonism
- Adjusted dosage when used concomitantly with levodopa or other parasympathetic inhibitors
- Optimal intake divided into 3 doses and taken at mealtimes
- Special caution for use in children
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with concurrent use of alcohol or other CNS depressants
- Potential intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk for tardive dyskinesia during concomitant administration of anticholinergics and neuroleptics
- Possible need for reduced dose of levodopa during concomitant therapy
Contraindications
- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma
Side Effects
- Dryness of the mouth, blurred vision, dizziness, mild nausea, nervousness
- Less troublesome reactions compared to belladonna alkaloids and unallenated parkinsonism
- Occasional suppurative parotitis, skin rashes, dilatation of the colon, paralytic ileus
- Psychiatric manifestations such as delusions, hallucinations, and paranoia
- Cognitive dysfunctions, constipation, drowsiness, urinary hesitancy, tachycardia, increased intraocular pressure
- Exacerbation of parkinsonism and neuroleptic malignant syndrome with abrupt treatment withdrawal
Pregnancy & Lactation
- No controlled data in animal or human pregnancy
- Caution required in lactating women due to potential suppression of lactation
Precautions & Warnings
- Gonioscope evaluation prior to therapy initiation
- Close monitoring of intraocular pressures
- Caution in hot weather, especially when given with other atropine-like drugs
- Possibility of hyperthermia with evidence of anhidrosis
- Reduced dosage to avoid impaired ability to maintain body heat equilibrium via perspiration
- Potential occurrence of Neuroleptic Malignant Syndrome with dose reduction or discontinuation
Overdose Effects
- Rare cases of death associated with Tridyl overdoses in conjunction with other CNS-depressant agents
- Central anticholinergic syndrome
- Peripheral signs of parasympathetic blockade
- Neuropsychiatric signs such as delirium, confusion, seizures
- Symptomatic and supportive therapy for acute overdose
- Gastric lavage or limit absorption
- Respiratory support and fluid volume replacement
