Description
- Exerts direct inhibitory effect upon the parasympathetic nervous system
- Relaxing effect on smooth musculature
- Inhibitory effect upon the parasympathetic nervous system
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for central muscarinic receptors in the cerebral cortex
- Lower affinity for peripheral muscarinic receptors
- Modification of nicotinic acetylcholine receptor neurotransmission
- Enhanced dopamine release in the striatum
Dosage & Administration
- Individualized dosage
- Low initial dose with gradual increments
- Dosing adjustment based on patient reaction
- Need for atropine in postencephalitic patients
- Administration before or after meals based on patient reaction
- Dose adjustment for concomitant use with levodopa or other parasympathetic inhibitors
- Precautions for abrupt withdrawal of treatment
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensification of anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk for tardive dyskinesia
- Need for dose reduction during concomitant therapy with other medications

- Narrow angle glaucoma

- Gonioscope evaluation prior to therapy
- Monitoring of intraocular pressures
- Caution in hot weather and with atropine-like drugs
- Consideration of narrow angle glaucoma in case of blurred vision
- Anhidrosis and hyperthermia precautions
- Potential risk of Neuroleptic Malignant Syndrome
Overdose Effects
- Central symptoms of atropine intoxication
- Dilated and sluggish pupils
- Warm, dry skin
- Facial flushing
- Decreased secretions
- Elevated temperature
- Tachycardia and cardiac arrhythmias
- Neuropsychiatric signs
- Stupor, coma, paralysis, cardiac and respiratory arrest
