Indications

• Essential hypertension
• Chronic congestive heart failure
• Hepatic cirrhosis with ascites
• Swelling due to excess fluid retention (edema)
• Hyperaldosteronism
• Resistant edema associated with secondary hyperaldosteronism

Pharmacology

• Complementary mechanisms of action
• Additive or synergistic diuretic effect
• Inhibition of Na+/K+/2Cl- co-transporter
• Reabsorption of sodium, potassium, and chloride ions
• Potassium loss induction
• Inhibition of aldosterone action

Dosage & Administration

• Furosemide 20 and spironolactone 50 mg tablet
• Furosemide 40 and spironolactone 50 mg tablet
• Dosage range: 20 to 80 mg of Furosemide
• Dosage range: 50 to 200 mg of spironolactone
• Suitability for elderly patients

Interaction

• Increased risk of hyperkalemia with ACE inhibitors or potassium salts
• Elevated levels of cardiac glycosides
• Possible reduction of diuretic effects by corticosteroids
• Potential increase in ototoxicity with aminoglycoside antibiotics
• Impact of sucralfate on the natriuretic and anti-hypertensive effect

Contraindications

• Anuria
• Acute renal insufficiency
• Severe impairment of renal function
• Hyperkalemia
• Addison's disease
• Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

• Increase in blood-urea-nitrogen concentrations

Pregnancy & Lactation

• Transfer of Spironolactone and its metabolites during pregnancy
• Fetal abnormalities with Furosemide use
• Consideration of contraceptive measures in women of childbearing age
• Metabolites detection in breast milk
• Discontinuation of breast-feeding if essential treatment is needed

• Caution with electrolyte deficiency
• Careful use in diabetes
• Enlarged prostate
• Hypotension
• Hypovolemia
