Description
- Neoclomide is a cytotoxic chemotherapy indicated for the treatment of various malignancies such as malignant lymphomas, Hodgkin's disease, multiple myeloma, leukemias, mycosis fungoides, neuroblastoma, adenocarcinoma of the ovary, retinoblastoma, carcinoma of the breast, and nonmalignant disease like biopsy proven 'minimal change' nephrotic syndrome in children.

Pharmacology
- Cyclophosphamide is biotransformed principally in the liver to active alkylating metabolites by a mixed function microsomal oxidase system
- Interfere with the growth of susceptible rapidly proliferating malignant cells
- Well absorbed after oral administration with a bioavailability greater than 75%
- Elimination half-life of 3 to 12 hours
- Eliminated primarily in the form of metabolites, but from 5 to 25% of the dose is excreted in urine as unchanged drug
Dosage and administration
- Initial course usually consists of 40 to 50 mg/kg given intravenously in divided doses over a period of 2 to 5 days
- Other regimens include 10 to 15 mg/kg given every 7 to 10 days or 3 to 5 mg/kg twice weekly
- Oral cyclophosphamide dosing usually in the range of 1 to 5 mg/kg/day for both initial and maintenance dosing
- Dosages must be adjusted in accord with evidence of antitumor activity and/or leukopenia
- Dialyzable for both drug and metabolites
- No consistent evidence indicating a need for dosage modification in patients with renal function impairment
Interactions
- Metabolism and leukopenic activity reportedly increased by chronic administration of high doses of phenobarbital
Contraindications
- Severely depressed bone marrow function
- Previous hypersensitivity to cyclophosphamide
Side effects
- Nausea, vomiting, anorexia, abdominal discomfort, diarrhea
- Hemorrhagic colitis, oral mucosal ulceration, jaundice
- Alopecia, skin rash, pigmentation changes, nail changes
- Leukopenia, thrombocytopenia, anemia with possible recovery after reducing the drug dose
- Hemorrhagic ureteritis, renal tubular necrosis
- Interstitial pneumonitis, interstitial pulmonary fibrosis, anaphylactic reactions, death
- Possible cross-sensitivity with other alkylating agents, syndrome of inappropriate ADH secretion, malaise, asthenia

- Special attention needed in patients with leukopenia, thrombocytopenia, tumor cell infiltration of bone marrow, previous X-ray therapy, previous therapy with other cytotoxic agents, impaired hepatic function, impaired renal function

- Insufficient data for patients 65 years of age and older
- Elderly patients may be more susceptible to Neoclomide toxicities
Overdose effects
- No specific antidote known
- Management with supportive measures for concurrent infection, myelosuppression, or cardiac toxicity if it occurs
