Indications
- Treatment of essential hypertension
- Management of chronic congestive heart failure
- Relief from swelling due to excess fluid retention (edema)
- Treatment of hepatic cirrhosis with ascites
- Management of hyperaldosteronism
- Relief from resistant edema associated with secondary hyperaldosteronism
Pharmacology
- Combination of Spironolactone (potassium sparing diuretic) and Furosemide (loop diuretic)
- Complementary mechanisms and sites of action
- Additive or synergistic diuretic effect
- Inhibition of Na+/K+/2Cl- co-transporter in ascending Loop of Henle
- Blockage of reabsorption of sodium, potassium, and chloride ions
- Increase in urination quantity and sodium excretion
- Reduction of potassium loss
Dosage and Administration
- Furosemide 20 and spironolactone 50 mg tablet
- Furosemide 40 and spironolactone 50 mg tablet
- Dosage range of 20 to 80 mg of Furosemide and 50 to 200 mg of spironolactone
- Increased dosage for previously stabilized patients
- Inappropriate for use in children
- Slower excretion in the elderly
Interactions
- Increased risk of hyperkalemia with ACE inhibitors or potassium salts
- Enhanced levels of cardiac glycosides like digoxin in the blood
- Potential reduction or abolition of effects when used with indomethacin or other NSAIDs
- Possible increase in ototoxicity of aminoglycoside antibiotics
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function
- Hyperkalemia
- Addison's disease
- Hypersensitivity to Spironolactone, Furosemide, or sulphonamides
Side Effects
- Headache, drowsiness, and gastrointestinal distress
- Ataxia, mental confusion, and skin rashes
- Gynaecomastia and breast enlargement
- Endocrine disorders including hirsutism, voice deepening, and impotence
- Transient increase in blood-urea-nitrogen concentrations and mild acidosis
- Hyponatremia, hyperkalemia, and dehydration
- Possible vascular thrombosis and embolism
Pregnancy and Lactation
- Crossing of spironolactone and its metabolites through placental barrier
- Animal teratology studies indicating potential fetal abnormalities with Furosemide
- Use considerations in pregnant women based on benefit versus hazard assessment
- Detection of Spironolactone and Furosemide metabolites in breast milk
- Recommendation for alternative method of infant feeding if essential to use Spironolactone
- Excretion of Furosemide in breast milk and need for discontinuation of breastfeeding if treatment essential
Precautions and Warnings
- Caution in patients with electrolyte deficiency, diabetes, enlarged prostate, hypotension, and hypovolemia
