Pharmacology
- Active form is Calcitriol
- Binds to Vitamin D receptors
- Half life of about 50 days
- Absorbed in the small intestine
- Metabolized to 25-hydroxy Vitamin D3 in the liver
- Second hydroxylation occurs in the kidney
- Increases calcium absorption
Dosage & Administration
- 40000 IU once weekly for 7 weeks for adults with Vitamin D3 deficiency
- 1400-2000 IU/day for maintenance therapy in adults
- 20000 IU every 4 weeks for prevention of Vitamin D3 deficiency in adults
- 20000 IU once a month as addition to specific therapy for osteoporosis in adults
- 20000 IU once every 2 weeks for 6 weeks for children with Vitamin D3 deficiency
- 20000 IU every 6 weeks for prevention of Vitamin D3 deficiency in children
- 1000 IU (1-2 tablets) daily for film-coated tablet
- 1000-2000 IU daily for oroflash or chewable tablets
- 400 IU/day for patients with risk of Cholecalciferol deficiency (0-1 yr)
- 600 IU/day for patients with risk of Cholecalciferol deficiency (>1 yr)
- 100000-200000 IU every 6 months for specific populations via injection
- 1 ampoule (1 ml) i.e. 200000 IU for Vitamin D deficiency in adults via injection
Interaction
- Interference with absorption and metabolism by various drugs
- Increased need for Vitamin D3 with Corticosteroids
Contraindications
- Contraindicated in diseases associated with hypercalcemia
- Contraindicated in hypersensitivity to Vitamin D3
- Contraindicated in evidence of Vitamin D3 toxicity
