Indications
- Treatment of adults with type 2 diabetes mellitus
- Adjunct to diet and exercise
- Insufficiently controlled on maximally tolerated dose of Metformin alone
- Combination with other medicinal products for the treatment of diabetes
- Already being treated with the combination of Empagliflozin and Metformin as separate tablets
Pharmacology
- Inhibitor of Sodium-Glucose Co-Transporter 2 (SGLT2)
- Reduces renal reabsorption of filtered glucose
- Lowers the renal threshold for glucose
- Increases urinary glucose excretion
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Should be individualized based on effectiveness and tolerability
- Take twice daily with meals
- Gradual dose escalation to reduce gastrointestinal side effects
- Maximum recommended daily dose of Metformin Hydrochloride 2000 mg and Empagliflozin 25 mg
- Starting dose individualized based on the patient's current drug regimen
- Extended-release formulations available
- Renal impaired patient assessment required
- Not recommended for pediatric patients under 18 years of age

- Hypersensitivity to Empagliflozin and Metformin
- Acute metabolic acidosis
- Diabetic pre-coma
- Severe renal failure (GFR <30 ml/min)
- Acute conditions with potential to alter renal function
- Disease causing tissue hypoxia
- Hepatic impairment
- Acute alcohol intoxication or alcoholism

Overdose Effects
- No toxicity in single doses up to 800 mg Empagliflozin
- No hypoglycemia seen with Metformin doses up to 85 g
- Lactic acidosis occurred with high Metformin doses
- Medical emergency requiring hospital treatment
- Haemodialysis for lactate and Metformin removal
