Indications
- Essential hypertension
- Chronic congestive heart failure
- Hepatic cirrhosis
- Swelling due to excess fluid retention
- Hyperaldosteronism
- Resistant edema associated with secondary hyperaldosteronism
Pharmacology
Spironolactone and Furosemide have different but complementary mechanisms and sites of action. When given together, they produce additive or synergistic diuretic effects.
Dosage & Administration
- Furosemide 20 and spironolactone 50 mg: 1 to 4 tablets daily
- Furosemide 40 and spironolactone 50 mg: 1 to 2 tablets daily
- Not suitable for use in children
- May be excreted more slowly in the elderly
Interaction
- Increased risk of hyperkalemia with ACE inhibitors or potassium salts
- Spironolactone increases levels of cardiac glycosides
- Corticosteroids may cause hypokalemia when used with Spironolactone
- Furosemide may increase the ototoxicity of aminoglycoside antibiotics
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function
- Hyperkalaemia
- Addison's disease
- Hypersensitivity to Spironolactone, Furosemide, or sulphonamides
