Indications

  • Treatment of type 2 diabetes mellitus
  • Used in adults, children from 10 years of age, and adolescents
  • May be used as monotherapy or in combination with other oral antidiabetic agents or with insulin

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production

Dosage & Administration

  • Available in immediate release tablet and extended release tablet forms
  • Dosage individualized based on effectiveness and tolerance
  • Maximum recommended daily doses should not be exceeded
  • Dosage increases should be made in increments

Interaction

  • May interact with Furosemide and Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • Certain drugs may produce hyperglycemia and affect glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Acute or chronic diseases causing tissue hypoxia
  • Acute alcohol intoxication

  • Substantially excreted by the kidney
  • May lower vitamin B12 levels
  • Increased risk of hypoglycemia when used in combination with insulin
  • Regular monitoring of renal function advised

Use in Special Populations

  • Dosage adjustments for elderly subjects based on renal function
  • Effect on growth and puberty in children to be carefully monitored

Overdose Effects

  • Hypoglycemia not seen with doses up to 85 gm
  • Lactic acidosis is a medical emergency
