Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis pain and signs of inflammation
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active COX-2 specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms with decreased potential for GI toxicity and effects on platelet aggregation
Dosage & Administration
- Recommended dose for Osteoarthritis: 30 mg once daily or 60 mg for insufficient relief
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily for 8 days
- Recommended dose for Postoperative dental surgery pain: 90 mg once daily, limited to a maximum of 3 days
- Shortest duration possible and the lowest effective daily dose should be used to minimize cardiovascular risks
Interaction
- Interacts with Oral anticoagulants, Diuretics, ACE inhibitors, Angiotensin II Antagonists, Acetylsalicylic Acid, Ciclosporin, tacrolimus, and Lithium
- Take without regards to meals
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active GI bleeding
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Pregnancy, lactation, and children under 16 years of age
- Inflammatory bowel disease, Congestive heart failure, Hypertension, Ischaemic heart disease, Peripheral arterial disease, and Cerebrovascular disease
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Atrial fibrillation, transient ischaemic attack, chest pain
- Flushing, cough, dyspnoea, epistaxis, anxiety
- Impaired mental acuity, paraesthesia, electrolyte disturbance
- Myalgia, arthralgia, confusion, hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not known whether excreted in human milk - not recommended for breastfeeding women
Precautions & Warnings
- Caution with patients at risk of developing GI complications with NSAIDs
- Caution with patients at risk of cardiovascular events
- Monitoring of renal function in certain patients
- Exercise caution in patients with cardiac failure, edema, liver dysfunction, and signs of hypersensitivity
- May mask fever and other signs of inflammation
- Consideration needed when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed with single or multiple doses up to 150 mg/day
- Usual supportive measures to be employed in case of overdose
